Health App
For Sun Life voluntary life insurance, a health app must be filled out on Sun Life's website. Below are the step by step instrucitons. Failure to complete the process will result in your life insurance being cancelled.
Go to: https://www.sunlife.com/us/en/ and click Sign In.
Click on Create an account.
Enter name, email, then click Continue.
You will receive a verification email. Check your SPAM or Promotional folder.
Click on Verify my email.
Select Member and then click Continue.
Select Submit an EOI. Go back to your enrollment link and in the Voluntary Life section you will find the Policy Number. Type it in (do not copy and paste), then click Continue.
Enter your name and create a Username and Password. Then click Continue.
Create your Security Questions and Answers.
Select the Agree boxes and click Finish Registration.
You are not registered and must click Sign in to continue. Enter your Username and Password and click Log in.
Once logged in, click on Submit Evidence of Insurability. Then complete the prompts and you will be able to finish the application.
